Survey to gauge awareness of narcolepsy & cataplexy among non-sufferers

I've created a very short survey (as part of the narcolepsy / cataplexy education focused Facebook group) that I'd like to ask people to share amongst non-sufferers wherever they can- Facebook, twitter, wherever people feel comfortable sharing the link. There are only five, very short questions, which should only take a few minutes of someone's time to complete. A few minutes that could help further ascertain what the current assumptions are that surround narcolepsy / cataplexy. Results so far have been interesting and a little surprising.
